Anti 3d Crash

It was based in the existing "Emailer's nod3dcrash"
mutator, but it uses less cpu time and does not produce
access none errors. Also does ban instead of kick.

Completely server side.

Install:
-Setup as mutator "anti3dcrash.anti3dcrash"
